Kapi‘olani Community College and the Human Resources Office will be offering Flu shots to ALL employees and students at ‘Ilima 202B on Wednesday, September 5, from 12:30pm – 2:30pm and Thursday, September 6, from 12:30pm – 2:30pm.
Reserve your spot today at: https://form.jotform.com/82066870335156. Space is limited. Please be sure to bring your picture ID, insurance card, and completed Vaccine Consent Form (download from TimePharmacy.com) on the day of your vaccination.
Cost of Flu shot will be billed to your medical insurance, or as an out-of-pocket cash or check payment)
