2021 Conflict of Interest (COI) Forms

Please see attached the Chancellor’s Memo regarding the annual Conflict of Interest Disclosure forms. The Department/Unit Heads are requested to forward the Memo and COI forms to all the employees in your department. Required by UH Executive Policy E5.214, this form must be completed by all employees (defined in A5.504 employee means any person possessing either a full-time or part-time appointment at the University, whether compensated or not, including positions of the Research Corporation of the University of Hawai‘i (RCUH)).

Part I and Part II (if applicable) must be completed, signed, dated, and given to the supervisor for review and signature. A physical/wet or digital signature is accepted for COI forms.
Please ensure all the employees complete:

  • All the questions of Part I; signs and dates; along with the supervisor’s certification. (Note: Question #4 is a two-part question that is frequently left unanswered)
  • Part II only needs to be completed if your employee answered ‘yes‘ to any of the Part I questions.
 A tracking sheet has been prepared for each department to assist in tracking the completion of the forms and they will be emailed out separately to each departmental contact.

$2000 Student Summer Research Opportunity – Due March 22

Are you interested in getting PAID while gaining research experience? Kapi’olani Community College (KapCC) and Chaminade University (CUH) Title III partnered together to create a 10-week research internship program just for you. Research internships are a great way for you to boost your scholarship applications, find out what you want to do, and potentially get a better letter of recommendation. 

Summer Research Institute (SRI)

Program Dates: May 24 – August 6, 2021

Stipend: $2000

Students will work with a faculty mentor on an interdisciplinary project based on Native Hawaiian culture, ‘āina-based, or sustainability topics. Past students worked with faculty in English, history, HOST to research sustainable volunteer tourism, Native Hawaiian resilence during COVID-19, and how to put ʻulu on people’s dinner tables.  

Students from any discipline can apply, but preference is given to non-STEM students. Eligible students must meet the following criteria:

  • Be a sophomore at KapCC in Fall 2021;
  • Have at least a 2.5 GPA on a 4.0 scale;
  • Have at least one semester of undergraduate education remaining after completing the summer research institute.
  • Must be able to commit 20 hours/ week (includes doing independent research and can be a little flexible)

Native Hawaiian and Pacific Islanders are highly encouraged to apply. Peer mentors from the Ka Hiki Mai Scholars program are available to help you with your application. Schedule a virtual or in-person appointment HERE. In-person appointments will be at Chaminade and social distancing rules will be enforced.

Just join via the link below, and feel free to munch on your lunch while you peek into the classes (and teacher minds!) of your colleagues. These Zoom sessions are intended to be informal, and there will be time for conversation and Q&A.

Two desserts, many forks! At lunchtime (12:00 – 1:00 pm) every Friday this semester, please join your Kapi‘olani CC colleagues for a fun lunch-hour chat and tour of online classes. Each Friday, two teachers will give us a tour of one of their classes and share with us their sweetest ideas. These informal Zoom sessions will include time for Q&A and conversation. TOMORROW: Walk-A-Thon!

WALK-A-THON: Friday, February 26, 3:00 PM – 5:00 PM, Great Lawn

Come and join the Board of Student Activities (BOSA) on campus for a fun, socially-distanced Walk-A-Thon! 

This event is dedicated to spreading knowledge about Cancer Awareness Month. We will have a competition for prizes for individuals with the most steps walked, fun games set up on the Great Lawn, info tables with several organizations, and lots of snacks. There will also be a station dedicated to making cards for essential workers during COVID as well as to support the kupuna community. Bring water, wear a facial covering, and get ready to walk! We hope to see you there!
